SIGN IN TO THE PATIENT PORTAL

The patient portal is where you share information, establish an appointment and interact directly with our physician via live stream video. We utilize a HIPAA compliant telemedicine software to protect your confidential medical information. In order to begin, you will go to the a patient portal tab and create your profile.

YOUR INFORMATION

Your profile is created when you complete the questions, sign and upload additional health information for our physician to review.

SCHEDULE CONSULTATION

You will be contacted to set up an appointment by our scheduling coordinator.

LABS AND OTHER TESTING

During the consultation, your history and physical will be reviewed to determine if BHRT is a suitable therapeutic intervention. If you are a candidate, we will request certain lab(s) and additional testing to further clarify treatment recommendations.

REVIEW OF LAB RESULTS

Together, you and our physician will review the lab results and develop an individualized BHRT treatment plan.

1 MONTH FOLLOW-UP

This is a brief consult to assess your progress and answer any questions or concerns you may have.

3 MONTH FOLLOW-UP

This consult is used to assess the effects of BHRT and if any changes need to be made to further address persistent symptoms.

6 MONTH FOLLOW-UP

Our physician will review goals of therapy and make additional adjustments as needed.

MAINTENANCE PLAN

Once hormonal balance is achieved, a maintenance plan will be developed. During maintenance, there will be required annual labs and a consult every 6 months.
